| St. Joseph County Fair 4-H Animal Affidavit | County wide form. One of these forms is needed for each species
of animal that is going through the 4-H Auction per child. (ex. If you have 2 children each putting a goat and
a duck through the auction then you need FOUR of these forms). This is the form where you indicate if the
animal(s) have or have not received any medications in the last 30 days. |
